The union dues/ service fee authorization form can be downloaded in either of two formats:

authorization form in PDF format (requires the free Adobe Reader.)

authorization form in rich-text format (requires MS-Word or other word-processor.)

Print out the form, fill in your information, and mark whether or not you wish to be a LEO member. Then mail the form to the address below.
